PPO Plan: Guardian and its subsidiaries have over 124,000 dentists in over 363,000 locations*. Chances are your dentist is in our network.

DHMO Plan: Guardian and its subsidiaries have over 12,000 dentists in over 52,000 locations from which you and each family member can select a primary care dentist.

  1. With Guardian’s Group Dental PPO Insurance, you and your broker can configure a plan with features that best meet your needs. Our dental PPO is available to companies of any size — starting at just 2 employees — whether benefits are employer-paid or voluntary.
  2. PPO Plan: Guardian and its subsidiaries have over 124,000 dentists in over 363,000 locations. Chances are your dentist is in our network.   3. Your plan might limit the amount of coverage you can receive in a year through an annual maximum for dental services and procedures. People who obtain dental insurance with Guardian can elect a Maximum Rollover benefit option for their dental PPO plan.

In some states, you may be able to choose between a PPO and DHMO plan. With a PPO plan you can see any dentist, but save more when you visit a dentist in your network. A DHMO plan typically costs less, but you need to see the dentist you selected from the network.
